DEPOSIT POLICY: A non-refundable $75 deposit is required to reserve first visit appointments including, but not limited to, Initial Assessment (45-60 minutes)

PLEASE NOTE: Every credit card transaction now includes a 4% add-on fee for staff training and retention. Thanks for your patronage and continued support as we strive to build healthier lives for each person who chooses to receive care with us.

Cancellation Policy

We require a minimum of 24 hours’ advance notice to cancel or change any appointment.  Initial appointments canceled with less than 24 hours’ notice may incur a $40 fee only if we are unable to fill the appointment

We understand that life happens – emergencies or unforeseen circumstances may arise throughout the day. In these instances, Agasar Family WellCare may consider a one-time waiver of fees with less than 24 hours’ notice. However, we kindly request the courtesy of a phone call giving us as much advanced notice as possible.
